Issued by LPMD of CA, PC, a California professional medical corporation, which provides your medical care. Nurvana Aesthetics & Wellness, Inc. provides administrative and technology services to the practice and handles health information as its business associate under a written agreement.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment

We are required by law to protect the privacy of your health information, to give you this notice describing our privacy practices, and to follow the terms of the notice currently in effect. We are also required to notify you if a breach affects your unsecured health information.

This notice covers your health information. How this website handles general visitor data — cookies, analytics, contact forms — is a separate matter, covered by our Privacy Policy.

How we may use and disclose your health information

Treatment

To provide, coordinate and manage your care. For example, a provider may review your intake and laboratory results to determine whether a medication is appropriate, and may share your prescription with the pharmacy that fills it.

Payment

To bill and collect for services — for example, to process your card payment, or to confirm the cost of a medication before it is charged.

Health care operations

To run our practice: quality review, provider evaluation, training, licensing, audits and business planning.

Business associates

We use vendors who handle health information on our behalf, including our electronic health record, telehealth, payment, laboratory, pharmacy-routing and communications providers. Each is bound by a written agreement requiring them to protect your information.

Appointment reminders and health-related communications

To contact you about appointments, refills, results and care instructions. Marketing messages are separate and are covered below.

Other uses permitted or required by law

Including: as required by law; public health activities; reporting abuse, neglect or domestic violence; health oversight activities; judicial and administrative proceedings; law enforcement; coroners, medical examiners and funeral directors; organ and tissue donation; research approved by an institutional review board; to avert a serious threat to health or safety; specialized government functions; and workers’ compensation.

Uses that require your written authorization

We will not use or disclose your health information for any purpose other than those described above without your written authorization. Your authorization is specifically required for:

  • most uses and disclosures of psychotherapy notes
  • marketing communications where we receive payment from a third party
  • any sale of your health information

You may revoke an authorization in writing at any time. Revocation does not affect disclosures already made in reliance on it.

Your rights

  • Access and copies. Inspect and receive a copy of your record, including electronically where we keep it electronically. We may charge a reasonable, cost-based fee.
  • Amendment. Ask us to correct information you believe is incorrect or incomplete. We may deny the request and will explain why in writing.
  • Accounting of disclosures. Receive a list of certain disclosures we made in the six years before your request.
  • Restrictions. Ask us to limit how we use or disclose your information. We are not required to agree, with one exception: if you pay in full, out of pocket, for a service, you may require us not to disclose information about that service to your health plan.
  • Confidential communications. Ask us to contact you at a specific address or by a specific method.
  • Paper copy. Receive a paper copy of this notice on request, even if you agreed to receive it electronically.
  • Breach notification. Be notified if a breach affects your unsecured health information.

Changes to this notice

We may change this notice and make the new terms apply to information we already hold. The current version is always posted on this page and is available at our office.
